For Xiomara Perez-Butler, Care Connect is more than a home care agency. It is the result of more than 25 years of experience working alongside individuals, families, caregivers, and healthcare professionals and seeing firsthand what families truly need when someone they love requires support at home.
Xiomara founded Care Connect with a simple belief: home care should feel personal.
Families deserve more than someone filling a shift. They deserve caregivers who are thoughtfully matched, communication they can depend on, a team that listens, and care that can evolve as their loved one’s needs change.
More Than 25 Years of Experience
Xiomara’s experience spans both medical and non-medical home care environments and includes home care operations, caregiver recruitment and training, scheduling, family communication, care coordination, and quality oversight.
Before founding Care Connect, she spent more than eight years successfully leading a non-medical home care franchise. Her professional background also includes experience within a medical visiting nurse agency, giving her a broad understanding of the challenges families may face as care needs change over time.
That experience shaped the kind of agency she wanted Care Connect to become: responsive, dependable, compassionate, and centered around the individual receiving care.
A Personal Approach to Leadership
Xiomara remains actively involved in Care Connect and believes leadership should remain connected to the people the agency serves.
Her approach emphasizes thoughtful caregiver matching, caregiver education, clear communication with families, ongoing quality follow-up, and making adjustments when a client’s needs or circumstances change.
She believes families should feel comfortable picking up the phone and knowing there is a team on the other end that understands their situation and is ready to help.
A Commitment to Dementia Education
Dementia and Alzheimer’s care hold an important place within Care Connect.
Xiomara has made caregiver education and community awareness a priority, helping caregivers, families, and professionals better understand what it may feel like to live with cognitive changes and how patience, dignity, communication, and person-centered care can make a meaningful difference.
This commitment extends beyond providing care in the home. Care Connect believes education can help families feel better prepared, caregivers become more confident, and communities become more understanding of individuals living with dementia.
