Care That Honors Their Service

Veterans have spent part of their lives serving others. When additional support becomes necessary at home, they deserve care delivered with the same values of respect, dignity, reliability, and independence. Care Connect, LLC provides personalized non-medical home care for veterans who need assistance with everyday activities while continuing to live in the comfort and familiarity of their own homes. Every veteran is different — our care is built around the individual’s routines, abilities, preferences, personality, household, and family support system, not simply around a diagnosis or list of tasks.

Personalized Support at Home

Some veterans may need just a few hours of assistance with everyday routines, while others may benefit from more consistent support throughout the week. Depending on the veteran’s needs and established care plan, Care Connect caregivers may assist with:

  • Bathing, dressing, grooming, and personal hygiene

  • Toileting and incontinence care

  • 󰨾

Mobility, transfer assistance, and walking safety supervision

  • Meal planning, preparation, and feeding assistance when appropriate

  • Hydration reminders and encouragement

  • Medication reminders

  • Light housekeeping and laundry

  • Grocery shopping and everyday errands

  • Companionship and social engagement

  • 󰊅

Assistance maintaining morning and evening routines

  • 󰨚

Accompaniment to appointments and community activities

  • Respite support for family caregivers

  • 󰞘

    Dementia and memory-care support

  • Fall-prevention awareness and general home safety

  • Reporting changes or concerns according to the established care plan

Supporting Independence, Not Taking It Away

Needing help at home doesn’t mean giving up independence. Our caregivers provide assistance where it is needed while encouraging veterans to remain involved in their own routines and decisions whenever possible — that may mean choosing what to wear, participating in meal preparation, continuing a favorite hobby, visiting friends, attending community activities, or simply maintaining the routines that have always been part of everyday life. Our role is to support the person, not take over their life.

Companionship & Meaningful Connection

For some veterans, the greatest need isn’t hands-on personal care — it’s having a dependable person there. A caregiver can provide conversation, share meals, take walks, accompany the veteran into the community, participate in favorite activities, or simply provide a reassuring presence at home. We take time to learn about the individual behind the care plan: their interests, experiences, preferences, routines, and what makes a good day meaningful to them.

󰞘 Support for Veterans Living With Dementia

Veterans living with Alzheimer’s disease, dementia, or other memory changes may require additional structure, supervision, reassurance, and support throughout the day. Care Connect caregivers can help maintain familiar routines, provide meaningful engagement, offer appropriate reminders and redirection, assist with personal care, and provide a calm and reassuring presence in familiar surroundings. Our approach remains centered on dignity, patience, safety, and meeting the individual where they are.

Helping Veteran Families, Too

Families often play an important role in helping veterans remain at home. Spouses, adult children, and other family caregivers may find themselves balancing personal care, household responsibilities, appointments, work, and their own families. Care Connect can provide an additional layer of support through scheduled home care or respite services, giving family caregivers time to attend to other responsibilities while their loved one continues receiving dependable assistance.

󰇗 Working Alongside the Veteran’s Healthcare Team

Some veterans receive services through physicians, visiting nurses, therapists, the VA healthcare system, or other healthcare providers. Care Connect provides non-medical home care and does not replace nursing, therapy, or other clinical services. When other professionals are involved, our caregivers can provide complementary non-medical support within the established care plan while communicating appropriate observations and concerns through the proper channels.

VA Community Care Network

Care Connect is a VA Community Care Network provider through Optum, allowing us to serve eligible veterans when home care services have been properly authorized. Eligibility and authorization for VA-funded services are determined through the appropriate VA and Community Care processes, and an authorization should be in place before VA-covered services begin. For veterans and families who already have an authorization, Care Connect can help review the approved home care schedule and coordinate next steps. Veterans may also contact us to discuss private-pay home care options when appropriate.

Care Built Around the Veteran

Service is part of a veteran’s story, but it isn’t their entire identity — they are parents, grandparents, spouses, neighbors, professionals, friends, and members of their communities. At Care Connect, we want to know the person we’re caring for: what matters to them, how they prefer their day to go, what they enjoy, what they value, and how we can support them without unnecessarily taking away independence. Quality home care should never feel like someone has taken control of your life. It should feel like you have the right support to continue living it.
